Learner Record – This Learner Record is used to demonstrate knowledge and understanding of key principles and skills underpinning person-centred planning and person-centered work in work with clients in a variety of areas – eg disability, community, working with people with dementia etc.
You are required to produce EIGHT separate journal entries reflecting the experience and detail of person-centered planning. Each journal entry can be between 300 and 500 words. Ensure confidentiality is maintained throughout if you are drawing on practice examples from a workplace or experience.
Each journal entry can be based around a specific activity with a client/clients (if you have experience to draw on) and/or also reflect learning from class. You can also use evidence from any of the documentaries we have looked at in class (eg, Aras Attracta, Carebright, Hoegewey etc, Sabine etc or any other examples and resources).
Learner Record 1:
Based on your experience in a practice context (if applicable) and coursework during the year , discuss:
- The challenges/barriers organisations face in meeting clients’ needs in a person-centred way
- Outline the different roles of individuals involved in supporting client/s – consider here members of the circle of support, advocates, professionals, etc

- Discuss the types of communication that were important in an interaction with a client or on a selected case study/video clip .
- Discuss the barriers to effective communication and how this impacts on person-centred planning

- Using the video material of Emily, discuss the key tensions that occurred in this case – Emily’s rights, desires and wishes versus safeguarding.
- Use the case study of Emily to discuss this and show how this organisation and staff helped to resolve this in a positive way
- How was Karen empowered to bring about positive change in this scenario?

- Discuss the importance of underpinning principles that encourage best practice – for example, the importance of supporting autonomy, choice, respect and dignity using examples of this in practice

- Discuss the process that should take place to draw up a one- page profile – eg the person is at the centre, the profile should discuss what is important to the person – use the video material to support your answer also.
- What resources are needed to ensure that the profile really reflect person-centered planning? (think here of time, staffing, organisational culture etc.
